Stavros Online New Hire Paperwork Portal

Stavros has partnered with PayPro to create an online portal where PCA new hire paperwork can be completed online. We are currently looking for 5 consumers that are hiring new PCAs to help us test the portal. If you are interested in being part of this pilot program, please contact us at 800-442-1185 or email fiscalfeedback@stavros.org. Consumers and PCAs will need to have their own email addresses and have a cell to receive text messages for security purposes.

New Hire Information

All newly hired PCAs are required to submit valid and completed government I-9 and W-4 forms along with a PCA Signature form, Non-Covered Services Attestation, Direct Deposit form (with proof of account) and New Hire form before their checks can be processed. Consumers also must provide their PCAs with copies of the Union Applications. New hire packets with all the forms necessary to complete the set up process can be found below.

Form I-9

This form is required. It is the federal employment eligibility verification form. Please make sure you provide the correct, unexpired documents for section 2.

Form W-4 (English)

This form is required. Use it to submit your federal tax withholding.

Form W-4 (Spanish)

This is the Spanish version of the federal withholding form.

Form M-4

This form is required. Use this form to submit your income tax withholdings for Massachusetts

Direct Deposit is required for all PCAs. Please make sure a proof of account from your financial institution is included with this form.

PCA Non-Covered Services Letter and Attestation

This form is required. This document outlines covered and non-covered services in the PCA program. Any non-covered services are not eligible for payment. This document also contains an attestation form that all newly hired PCAs must sign and submit with their new hire paperwork. The letter and attestation forms are in both English and Spanish.

PCA Orientation

Starting February 1, 2021, the Workers' Rights section of the orientation is being moved from the consumer led portion of the orientation to the 1199SEIU Training and Upgrading Fund portion along side the Infection Control training that started on October 1, 2020 . Any PCA completing the orientation with their consumer on or after February 1, 2021, will need to complete the final 2-hours of the orientation with the Homecare Training Benefit within 30 days of completing the orientation with their consumer. PCAs will be paid 4-hours for completing the orientation only after completing both portions of the orientation. Memos regarding these changes are below.

Debit Card Information

PCAs must have their paycheck direct deposited into a bank account or on to a debit card. Please click on the link below to download the Stavros FI Direct Deposit form. The direct deposit forms must be filled out completely and include documentation (for proof of account) from your financial institution. Please remember the consumer and/or surrogate cannot be on the account you wish to have you paycheck direct deposited to. Return completed forms to the Stavros FI office. US Bank Focus Blue Debit Card

The U.S. Bank Focus Blue Debit Card can be used by PCAs to receive their pay via direct deposit. Please see the informational materials below. To sign up for the card, please use the U.S. Bank Focus Blue application and return it to Stavros FI for processing.

I am a PCA. How do I get direct deposit?

You can download the direct deposit form and instructions by clicking the link below or you can call our Customer Service Department and request a form. You have to fill it out completely with the correct routing number, account number, the name and the four digit number of your employer. We also require supporting documentation from your financial institution for your direct deposit.

Did my check go direct deposit? (Mainly for the first time)

To verify if your PCA payment was deposited into your bank account on the Friday of your pay cycle, please call your bank first. At the same time, your employer will be getting the non-negotiable check voucher with your payroll information on it.

How do I stop my PCA's direct deposit?

Please have your PCA request this in writing. Make sure they include your (employer's) name and company code. They will then need to provide us a new account for direct deposit by completing a Stavros direct deposit form with proof of account. Direct deposit is mandatory for all PCAs.

How can I get more timesheets, W-4s, I-9s, PCA Signature forms, or your weekly schedules?

You can download them from our site and print them from home if you have access to a printer. If you do not have access to a printer, you can request to have forms mailed to you by calling our Customer Service Department.

How can I get copies of my pay stubs?

You can go to https://www8.paychoiceonline.com/fea/ to access your pay stubs online. Please contact our customer service if you have any issues accessing your pay stubs online. A guide to access pay stubs online can be found below.
