All newly hired PCAs are required to submit valid and completed government I-9 and W-4 forms along with a PCA Signature form, Non-Covered Services Attestation, Direct Deposit form (with proof of account) and New Hire form before their checks can be processed. Consumers also must provide their PCAs with copies of the Union Applications. New hire packets with all the forms necessary to complete the set up process can be found below.

Starting February 1, 2021, the Workers' Rights section of the orientation is being moved from the consumer led portion of the orientation to the 1199SEIU Training and Upgrading Fund portion along side the Infection Control training that started on October 1, 2020 . Any PCA completing the orientation with their consumer on or after February 1, 2021, will need to complete the final 2-hours of the orientation with the Homecare Training Benefit within 30 days of completing the orientation with their consumer. PCAs will be paid 4-hours for completing the orientation only after completing both portions of the orientation. Memos regarding these changes are below.

The direct deposit forms must be filled out completely and include documentation (for proof of account) from your financial institution. Please remember the consumer and/or surrogate cannot be on the account you wish to have you paycheck direct deposited to. Return completed forms to the Stavros FI office. The U.S. Bank Focus Blue Debit Card can be used by PCAs to receive their pay via direct deposit. Please see the informational materials below. To sign up for the card, please use the U.S. Bank Focus Blue application and return it to Stavros FI for processing.
